No one was injured in the so-called hard landing on Nov. 18, 2011, but the collision sent part of the dock to the bottom of the sea.
In a report released Wednesday, TSB investigators say the incident was due to a combination of mechanical shut downs and poor communication between the Queen of Coquitlam's captain and its chief engineer.
The report says the collision happened because the ferry was travelling too quickly to dock properly and the ship's bow propeller, which would have slowed the ship quickly, was out of service.
It was out of service because engineers had disengaged a clutch in order to work on a mechanical problem with one of its engines.
But the chief engineer didn’t tell the captain he had disengaged the clutch, and the captain didn't realize until it was too late.
