The 42-year-old woman is also facing charges of not providing a breath sample on demand and dangerous operation causing bodily harm, Ontario Provincial Police said Monday. The woman was also issued a 90-day driver's licence suspension.
Police say the Pontiac was heading southbound with three passengers inside when it crossed over into the northbound lane. It collided head-on with a full sized Chevrolet van carrying two adults and five children.
A child riding in the Pontiac and an adult in the van were both airlifted to a hospital in London with major injuries. The other eight people involved were taken to a local hospital.
Police say the crash remains under investigation.
