West Vancouver Police say the incident began when panicked 911 calls started flooding in shortly after 7 p.m. PT Wednesday.
The callers reported a white panel van was heading eastbound in the westbound lanes of the Upper Levels section of the highway near Taylor Way.
Shortly afterwards 911 callers reported a three-vehicle crash just west of the Capilano Bridge.
The panel van had smashed into two vehicles, including one minivan with a family of four, and a lone driver in another vehicle.
West Vancouver Police say a two-year-old girl in the minivan was taken to hospital and underwent surgery for a fractured shoulder. The rest of the family and the other driver did not appear to be injured, but were taken to hospital for assessment.
The driver of the panel van was also hospitalized with leg and hip fractures. Police say the driver was likely impaired.
Investigators have already spoken to many witnesses, but they are still asking anyone who saw the wrong-way van last night to give them a call at 604-925-7300.
